FREQUENTLY ASKED QUESTIONS (FQA) ON THE FORM3 DGT



Question 1: What is step 1 of the Form 3 DGT?

Answer to 1: First step is to accomplish the first process which is Option 1 in the F3DGT until all advisers with their grades and sections (advisory classes) are accounted and submitted. Accomplish only Option 1 if it is your first time to submit Form 3 data for the first month.

Question 2: What is step 2 of the Form 3 DGT?

Answer to 2: Second step is to accomplish the second process which is Option 2 in the F3DGT until all non-advisers and other non-teaching personnel of the school are accounted and submitted. Accomplish only Option 2 if it is your first time to submit Form 3 data for the first month.

Question 3: What is step 3 of the Form 3 DGT?

Answer to 3: You can already explore the portal and review the system-generated informations/outputs based on your inputs at the Form 3 Submission Portal.

Question 4: What is the Option 3 in the Form 3 DGT?

Answer to 4: It is the option to choose for the next or succeeding months to submit the CHANGES on monthly enrolment and attendance data of the AFFECTED grade and section ONLY. Choose Option 3 if it is your second or third time, etc., to submit changes on monthly enrolment and attendance data. Unless there are changes on advisers, no need to re-input names of advisers but the monthly enrolment and attendance data only in Option 3.

Question 5: Am I going to re-input monthly enrolment and attendance data monthly in Option 3 even if no changes on said data for the succeeding months?

Answer to 5: NO, you do not need to re-input monthly enrolment and attendance data for the succeeding months in Option 3 IF THERE ARE NO CHANGES ON ALL DATA OF THE GRADE AND SECTION ON MONTHLY ATTENDANCE OR ENROLMENT (i.e. no changes on the adviser and no changes on the grade and section MONTHLY ATTENDANCE OR ENROLMENT). In case no changes on said conditions, all you have to do is choose Option 4 only.

Question 6: What if there are changes on monthly enrolment or monthly attendance of one (1) grade level section only, am I going to re-input ALL grade levels and sections?

Answer to 6: No, no need to re-input ALL. Just re-input only the AFFECTED grade and section in Option 3 in the appropriate month when the change is in effect.

Question 7: How will I correct erroneous data submitted?

Answer to 7: Re-submit the affected grade and section with correct data and put remarks at the REMARK portion of the F3DGT that it is the correct data to replace the previous data submitted. The system will replace automatically the previous with your latest correct inputs.

Question 8: If an adviser handles multiple advisory classes or grades and sections, am I going to input his/her name in 1 advisory class only?

Answer to 8: You must input his/her name in ALL of the advisory classes or ALL of the grades and sections he/she is handling. Example, He/She is adviser in grade 1 section A and B and grade 2 section A and B, you must input his name in grade 1 section A and B and input also his name in grade 2 section A and B. But in the succeeding months, choose Option 3 only to input changes on enrolment or attendance of the affected grade and section.

Question 9: For the succeeding months of submitting Form 3 data, am I going to re-input the names of advisers with their monthly enrolment and attendance data?

Answer to 9: Unless there are changes on names of advisers, no need to re-submit names of advisers but choose Option 3 to submit the change on enrolment and attendance data only of the AFFECTED grade and section only.

Question 10: What option will I choose for an adviser who is on leave but with substitute?

Answer to 10: Choose Option 1 to input the name of the substitute teacher on the affected advisory class of the teacher being substituted for the appropriate month the substitution is in effect. At the REMARK portion of the F3DGT, put the name of the permanent teacher being substituted. When the substitution is over, choose Option 1 again to resubmit the name of the permanent teacher to his/her advisory class on the appropriate month the reinstatement to duty of the permanent teacher is in effect. After resubmitting the name of the permanent teacher to his/her advisory class, the system will automatically replace the name of the substitute teacher. 

Question 11: What will I do with combination classes?

Answer to 11: To determine enrolment per grade and section of combination classes, input the name of adviser to ALL grade levels affected by the combination in Option 1 and input appropriate remarks per grade level at the REMARK portion of the F3DGT that said grade is combined with other grade levels. Example. Grade 1,2,3 combination classes: Remark for grade 1 is "Combined with 2,3". Remark for grade 2 is "Combined with 1,3". Remark for grade 3 is "Combined with 1,2". Etc.

Question 12: What will I do if there are additional teachers or non-teaching personnel of the school in the succeeding months?

Answer to 12: Choose either Option 1 (if adviser or substitute) or Option 2 (if non-adviser or non-teaching staff or SH) in the F3DGT for the appropriate month the update is in effect.

Question 13: Is it necessary that I will I do the submission of data in Option 1 or 2 or 3 in one sitting?

Answer to 13: No, it is not necessarily in one sitting. You can do the submission by installment in any said options at your convenience within the deadline which is every 10th day of the month (Example: if data to be submitted is March, deadline of submission is April 10).

Question 14: Why is Junior and Senior HS not separated on the total?

Answer to 14: After all schools have complied with the Form3DGT, the system will be updated to address the issue.

Question 15: I am sure I have submitted on the Form3DGTool the complete grade and sections of our school but why are some grade levels and sections missing at the Submission Portal?

Answer to 15: You probably did not put the name of the adviser on the missing grade and section. Probably, you just input the enrolment data in Option 3 instead of Option 1 first because it is the first time. Per instructions on the form, the name of an adviser who handles many advisory classes or many grades and sections must be entered on every grade and section he/she handles; but in the following months or succeeding months, no need to re-input the names if no changes on advisories. If ever changes occur on advisories, just re-input ONLY the AFFECTED grade and section in Option 1 because you need to input the name of the new adviser and to specify the advisory class he/she will be handling.

Question 16: If I want to replace the name of an adviser and changes on monthly enrolment and attendance of his advisory class, what Option in the Form3DGT will I choose?

Answer to 16: Choose Option 1 ONLY and input the AFFECTED grade and section ONLY. After submitting your inputs in Option 1 of the Form3DGTool, the "Form3 Submission System" will automatically update the name, enrolment and attendance of the affected grade and section then displays your latest inputs at the "Form3 Submission Portal."

Question 17: When I click NEXT BUTTON after I finished typing the names in the boxes provided, it does not respond. What probably is the problem?

Answer to 17: Check below the boxes if there are red-colored messages. Follow those messages, if any, then try to click Next Button again. Another possible cause is very slow internet connection. Reload the browser or wait till it responds.

Question 18: How does the Form3 Submission System ensure that data are coming from authorized uploaders?
Answer to 18: The "Form3 Submission System" has a validation process for that concern. The "Form3 DGTool" will just accept inputs from uploaders then the "Form3 Submission System" will validate if it comes from an authorized uploader. Once the system has validated, it will display notification of its findings to the administrator. Subsequently, the data uploaded by unauthorized person, if any, will be disregarded and the DepEd email of the unauthorized uploader will be held accountable.
Question 19: Why did you not conduct training prior to implementation of the Form3DGTool?
Answer to 19: Considering our present situation this time of pandemic, the concerned has decided to create the "Form3 DGTool" in Google Form so that it will not need training on its use when implemented. Google form is easy to use and you are all familiar with Google form. Also, Google form can be accessed easily even with slow internet connection as long as there is internet access. If you notice the "Form3 DGTool," It is full of instructions in every question to direct you on what to do. All you have to do is read and understand the instructions if first time to fill the form.

  1. I can follow the instructions its just that we have poor signal to do the work on time. I even wait at night until late hours just to send my report.

    1. It is why Google form is used to gather data because it is fitting with our current internet problems at home and at school. At least google form can be accessed even with a slow internet connection as long as there is internet access.

    1. Surely, you can get used to it easily. Google form is surely familiar to almost all. It is just a matter of familiarizing yourself with the parts of the form and with the steps so that you won't have to read the instructions in the next submissions.

    1. if you want to update names or changes names of advisers, choose Option 1 then just submit the AFFECTED advisory class only. If non-advisers, choose Option 2.

    3. Sir, what option will I choose to add substitute teacher with an advisory class since I input her name in option 1 but I can't view in the portal. Kindly check if I need to change or not.
      And also sir if her substitution ends, what option will I choose so that her name will not be reflected in the portal.

Answer to 10: Choose Option 1 to input the name of the substitute teacher on the affected advisory class of the teacher being substituted for the appropriate month the substitution is in effect.
